  • Provide assessments for home health patients

We are seeking a dedicated and compassionate Registered Nurse to join our home health care team. In this role, you will be providing direct nursing care to our patients in their homes, ensuring they receive top-quality care in a comfortable and familiar environment. This is a permanent, full-time position that requires a unique blend of clinical expertise, patient education, and emotional support to help patients manage their health conditions.

Responsibilities

  • Conduct initial assessments of new patients, including taking medical histories and assessing health conditions and needs.
  • Develop and implement individualized care plans for patients, adjusting as necessary.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor and report changes in patients' conditions, such as adverse reactions to medication or treatment.
  • Provide patient education and counseling to help them manage their health conditions.
  • Coordinate with other healthcare professionals, such as doctors and therapists, to ensure optimal patient care.
  • Provide wound care and administer infusion therapy as needed.
  • Maintain accurate and detailed patient records, including documenting patient progress and any changes in condition.
  • Adhere to all home health care policies and procedures, as well as state and federal regulations.
  • Provide emotional support and compassionate care to patients and their families.

Qualifications

  • Current and valid RN license in the state of practice.
  • Minimum of 2 years of nursing experience, with a focus on home health, home care, outpatient care, acute care, infusion therapy, and wound care is preferred.
  • Strong clinical skills with the ability to react decisively in emergency situations.
  • Excellent communication and interpersonal skills, with the ability to provide education and support to patients and their families.
  • High level of compassion and patience, with a commitment to providing top-quality care to patients in their homes.
  • Ability to work independently, with excellent problem-solving skills and the ability to adapt to changing situations.
  • Proficiency with electronic medical records and medical software.
  • Valid driver's license and reliable transportation.
  • Physical ability to lift and move patients, as well as to perform tasks such as dressing wounds and administering infusions.
  • Current certification in CPR and First Aid.
  • Must pass background check and drug screening.
